Registered Nurse Community Care Coordinator
The Community Care Coordinator Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assess, plan, implement, and evaluate the effectiveness of the Veteran's care. Serve to support delivery of high quality, timely care to the Veteran receiving services in the community. Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
