DEPARTMENT SUMMARY: The Metabolic and Bariatric Surgery Coordinator at St. Charles Health System engages in a collaborative process between the inpatient and outpatient care teams in conjunction with the American College of Surgeons program standards. The MBS Coordinator serves as a program coordinator and liaison between multiple care providers, community clinics, and inpatient care area. The work is characterized by program oversight, collaboration with the MBS Medical Director and the MBS clinical reviewer (data abstractor).
POSITION OVERVIEW: The MBS Coordinator at St. Charles Health System serves as a point of contact for referring physicians, patients, and caregivers to accessing clinical and supportive care services offered within St. Charles Health System and the community. This RN collaborates and coordinates the delivery of quality, efficient and cost-effective health care services for patients inclusive of all providers and partners of the health care team. This position does not directly manage any other caregivers. This position may also serve as the MBS Clinical reviewer in a primary or secondary abstractor role.
ESSENTIAL FUNCTIONS AND DUTIES:
Collaborate with healthcare providers and community partners and the St Charles Health System to maintain an evidence-based, comprehensive program oversite. Patient interactions and interventions are focused on access to care and education.
Provides patient self-management support and tools with a focus on empowering the patient/family to build capacity for self-care.
Act as a patient advocate and liaison between patients, providers, specialists, hospital, post-acute care services, and community resources as well as facilitate with internal services and external agencies.
Coordinates care services with other healthcare providers to enhance bariatric program offerings and specific patient referrals.
Provide follow-up with providers or patients during care transitions. Completes timely follow-up providing education utilizing teach back methodology, and coordination of care.
Participate in multidisciplinary team conferences and participates in overall program evaluation of processes, identifies problems, and proposes/develops process improvement strategies to enhance program services.
Acts as a resource to clinics and acute care departments to help develop best practice and up to date education.
Represents St. Charles Health System (SCHS) and the MBSAQIP program at community events, state chapter meetings, and collaborative events as applicable to program.
Participates in professional development, including national and regional conferences pertaining to program development, quality initiatives and best practice.
The MBS Coordinator may also perform data entry and data analysis pertinent to the MBSCR data analysts role, including full case abstraction and complete ongoing training and recertification requirements. The MBS Coordinator can retrieve, and document long term follow up data on all patients and fulfill requests for data and outcome reports for the MBS Committee and MDQA presentations.

Provides and maintains a safe environment for caregivers, patients and guests
Documents patient interactions and education with proficiency in compliance with MBSAQIP standards.
Responsible for credentialing with the American College of surgeons by ensuring all elements of the MBSAQIP accreditation are followed.
Leads site visit preparation every three years for accreditation and collaborates with the Bariatric Medical Director to prepare the American College of Surgeons.
Assists the Bariatric Medical Director and bariatric steering committee in designing and completing one quality project annually as part of the MBSAQIP requirements.
Assists the Bariatric Medical Director and medical staff office in generating the agenda for the quarterly bariatric meetings and will report to the committee on the progress of the program as outlined in previous minutes, numbers of cases, readmissions, morbidity and mortality.
Assists the Bariatric Medical Director in quality reporting to various committees in the hospital system.

EDUCATION
Required: As required for RN Licensure
Preferred: BSN
LICENSURE/CERTIFICATION/REGISTRATION
Required: Current Oregon RN license. Ability to travel to business functions/meeting with clients and providers/attend trainings/meetings and all SCHS worksites required.

EXPERIENCE
Required: A minimum of 3 years clinic nursing experience related to specialty area. Previous experience with emphasis on disease management and patient education.
Preferred: Experience in case management, nurse navigation, and/or care coordination. Community Health experience.
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
PHYSICAL REQUIREMENTS:
