Job SummaryThe Clinical Nurse Care Manager (CNCM I) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I facilitates transitional care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction.
