Job DescriptionThis position requires expertise in the nursing process and uses critical thinking skills to plan and coordinate patient care. This role collaborates with other team members to identify Medicare patients appropriate for Initial and Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV. Assists with the triage of patient calls and completion of other clinical tasks as needed. Assesses patient healthcare needs that require additional follow-up by primary care provider and assists with scheduling appointments.
