Job Description
The Breast and Cervical Cancer Program (BCCP) Nurse is responsible for the coordination of the BCCP grant program. This individual will be responsible for patient navigation throughout the continuum of care for BCCP cancer screening services. The BCCP Nurse will interact with contracted providers as well as the Harford County Health Department and works collaboratively with inter-disciplinary staff and partners. The nurse will screening and pathology results; consult with screening provider as needed and report if the recommended recall does not follow BCCP grant requirements. Coordinates clinic schedule, staffing, equipment and providers with BCCP secretary to meet program needs and goals. Provides follow-up and logistical support to individuals diagnosed with breast or cervical disease. Develops tracking tool to assist with data collection and reporting. Maintains quality improvement and ensures the clinic/activities follows the appropriate evaluation processes related to the grant program.

Qualifications
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:
Education & Training: Knowledge of nursing theory, practice and education in order to conduct patient triage; completes intakes and assessments and participates in the case management process. Knowledge of adult learning theory, instructional design principles, instructional media, evaluation methods and generally accepted training and development practices. Certification in Breast Navigation is required or must be obtained within 2 year of hire. BCLS Certification is required.
Work Orientation & Experience: Three to five years' experience in community nursing, health prevent and/or oncology services. Preferred advanced skill and knowledge of screening program and oncology software programs (CaST, Care-2-Care, Varian MO (Aria), and case management experience is preferred. The ability to implement sound educational programs is preferred. Current Maryland Registered Nurse license with experience in Oncology Practice, community health and preventative health.

JOB TASK LIST
- In collaboration with Director Oncology Clinical Programs, assists in the coordination of care for the BCCP, including but not limited to: triage, assessment, education, supportive care, referrals to specialty areas, and navigation per national guidelines. 2.
- Coordinates and manages department resources to achieve expected clinical outcomes and budgetary compliance.
- Focuses on continued development and enhancement of the BCCP to meet the changing landscape of healthcare and improve delivery of services to all eligible women.
- Responsible for development, implementation and on-going oversight of the BCCP grant work plan, guidelines and clinical standards5.
- Serves as a liaison between the contracted provider and clients by verifying the process of scheduling appointments and that follow-up is in accordance with grant and governing bodies' guidelines.
- Contacts patients to review screening results and assists with referrals, follow-up scheduling as needed. Ensures the patient receives appropriate letter/communication at specified intervals of service.
- Responsible for developing proficiency with required computer software programs. Assist secretary with reporting data and quality outcomes. Works closely with the HCHD to review program, outcome data, and updates related to BCCP grant.
- Coordinates any care issues identified by intake, screening, assessment, screening results and provider recommendations. Provides direct comprehensive and innovative nursing education, support and care to clients and their families.
- Provides all necessary education and support materials to clients in the BCCP. Uses motivational supportive and educational counseling to ensure commitment to screening guidelines and advocates for continued participation in the BCCP .
- Maintains the performance improvement/Service Excellence activities of the program. Assures quality patient care and adherence to program standards of care.
- Oversees the BCCP grant requirements and assist with monthly meetings, reporting data, quality monitoring and error tracking/correction within required databases. Analyzes data and program outcomes, reports monthly to Director of Oncology Clinical Programs and HCHD Cancer Prevention Program Manager.
- Participates in related professional educational and program requirements.
