Nurse Practitioner - Home Based Primary Care
The Home Based Primary Care (HBPC) Nurse Practitioner serves as a care manager and provides care to Veterans in their home within the assigned catchment area. Incumbent must live within the commuting area of the clinic/facility they are assigned to: Cumberland County VA Clinic or Lancaster County VA Clinic. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
