The role you’ll contribute: General Summary Description (Taleo)
The APRN/PA supports the clinical operation of the Transplant Institute with primary focus on clinical services in the inpatient and outpatient setting. Delivery of patient care is conducted in collaboration with the transplant surgeons and physicians including, but not limited to, physical exams, diagnosis and treatment, ordering and interpreting tests, counseling on preventative health, writing prescriptions and assisting in surgery. In addition, the APRN/PA is responsible for coordinating and managing the patient care of high-risk patient populations throughout the continuum to optimize the patient’s clinical, functional and psychosocial status, to provide the communication link for coordination of care among the multidisciplinary healthcare team involved with this patient type, and to orchestrate a performance improvement program that focuses on improving patient outcomes. Under administrative guidance and physician supervision, provides comprehensive primary care services, acute care services and extended care services in the area of nursing and/or medicine for individuals and their families. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Facilitates and coordinates evaluations for potential organ recipients or living donors which includes, but is not limited to the following:
Identifying appropriate candidates upon referral
Reviewing patient medical records/data and identification of co-morbidities/ contraindications to transplant (or living donation)
Arranging for necessary testing, exams and interviews of transplant candidates and compiling data for assessment.
Coordinating with the Financial Coordinator to obtain authorization for pre-transplant patient’s evaluation. This includes communicating as necessary with insurer case manager
Developing and instituting candidates’ plan of care
Obtaining necessary consent for transplant evaluation in accordance to United Network of Organ Sharing (UNOS) guidelines
Maintaining communication with referring physicians, healthcare providers and payers
Evaluates and treats patients with acute, chronic complaints and health maintenance concerns which include the following:
Assessment of patient health status through physical exam, reviewing patient history, diagnostic tests and procedures and reviewing labs and other critical data.
Diagnosis of acute and chronic conditions affecting all organs systems
Initiating orders for medications, laboratory and other diagnostic testing and medical treatments on appropriate forms
Prescribing interventions and performing therapeutic measures to ensure the appropriate individualized plan of care is implemented. Applying knowledge of age-specific needs, i.e., pediatric, adolescents, adult and geriatric patients related to plan of care interventions and patients’ responses to care
Evaluating patient’s mental status, substance abuse and referring to mental health treatment as clinically indicated
Assessing patient’s compliance and their adherence to medication regimen, treatment recommendations and lifestyle behaviors modifications
Assessing clinical factors including, but not limited to, immunosuppression complications, drug toxicity, infections complications and risk rejection
Assessing patient’s functional status
Assessing patient’s caregiver and transportation plan
Coordinating care of recipient and/or living donor as needed
Communicating frequently with referring physicians regarding the patient's status
Providing on-call coverage for medical triage for outpatients in collaboration with the physician on-call.
Implementing changes in drug treatment therapy as ordered by the physician(s) and/or healthcare providers
Call responsibilities determined by program-specific needs
Qualifications
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current BLS and ACLS certification
If Nurse Practitioner:
Current, valid State of Florida license as an Advanced Practice Registered Nurse
National Nurse Practitioner certification by either:
Current, valid State of Florida license as a Physician Assistant-C
(The temporary state license issued to the graduated Physician Assistant is considered a valid/ current license until such time as the Physician Assistant is a PA-C.)
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
If Nurse Practitioner or Physician Assistant (applies to both):
Clinical Coordinator Transplant Certification (CCTC) by the American Board of Transplant Certification (ABTC), or
Certification from a clinical research professional organization, i.e., SOCRA, ACRP or
Certification from the America Association of Heart Failure Nurses (AAHFN)
If Nurse Practitioner (applies only to NP), certification issued by either:
the National Certification Corporation (NCC) or
the Pediatric Nursing Certification Board (PNCB)
