Registered Nurse- Home Based Primary Care (HBPC)
The Home Base Primary Care (HBPC) Registered Nurse position is located within Home and Virtual Care Services, Nursing Service, VA St. Louis Health Care System (VASTLHCS), St. Louis, Missouri. The HBPC RN is a professional Nurse who functions as a care manager for empaneled HBPC patients, and is responsible for assessing and continually reassessing their needs and those of their caregivers, and delivering nursing care in the home. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
