Position Description:

The RN, Nurse Navigator supports the Spine and Joint Replacement service line working under the direction of the Director of Rehabilitation Services. Furthermore, this position will work in collaboration with and support the inpatient Rehabilitation Clinical Coordination and other interdisciplinary service lines to ensure safe and effective patient care delivery and the achievement of desired patient outcomes for the spine and joint replacement population. This position is responsible for facilitation of nursing practice, implementation of clinical navigation services, and continuum of care outcomes in collaboration with nursing, rehabilitation practitioners, physicians and case management. Moreover, this position has direct accountability for training and coaching as needed, responsible for navigating patient’s care from pre-admission, pre-surgery education, hospitalization , to post-discharge follow up care and monitoring of patient in collaboration with the any and all healthcare team(s), to best meet the patients’ physical, emotional and spiritual needs.
Essential Functions and Responsibilities of the Job
- Demonstrates leadership in the coordination of patient care across the healthcare continuum ensuring progress towards meeting positive patient outcomes consistently.
- Navigates patients and family members in person, virtually, or via phone on end to end process to ensure patients and families are equipped with the knowledge and expectations before, during and after their spine or joint surgery.
- Utilizes critical nursing skills to manage care for patient case load effectively and consistently.
- Collects and analyzes data to promote advancement and enhancement of the Spine and Joint Replacement service line.
- Educate nursing and rehabilitation staff on current best practice in relation to Spine and Joint Replacement patients to ensure knowledge is maintained throughout the interdisciplinary care team.
- Works within the clinical information system and applies principles to patient documentation to ensure compliance is maintained throughout the patient care life cycle.
- Demonstrates knowledge and understanding of working towards building patient census, decreasing readmissions to hospital or Emergency dept, increasing patient satisfaction scores, Doctor engagement scores and working towards making MemorialCare Saddleback’s presence known as a world class provider for Spine surgery and joint replacement.
