We are seeking a full-time Registered Nurse for the Infectious Disease Clinic who possess good organizational skills which enable completion of work and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. The incumbent should be self- directed and possesses effective skills in clinical assessment, critical thinking, and problem resolution. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: - National Certification in area of responsibility. - Current AHA BLS - 3-5 years of clinical experience related to patient care unit of assignment. - Evidence of core training in specialty if applicable. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The RN Care Manager works in partnership with the assigned Infectious Disease Providers and other PACT team members to deliver effective disease and chronic care management for an assigned panel of patients.\nThe RN Care Manager Nurse II is responsible for managing complex patient situations, acts as patient advocate, and provides supportive assistance to all members of the healthcare team. The incumbent is a Registered Nurse who is responsible for the clinical, research and educational aspects of the assignment. Some of the major responsibilities for this position include, and are not limited to: Work in unison with fellow RN to maintain and manage case management needs of the ID clinic Works in unison with the staff and provider's in promoting a patient centered culture that assures that quality health care services are provided to the veterans promptly, safely, courteously, and compassionately and that their significant others and/or caregivers are appropriately involved. Participates in the activities, goals, and functionality of the Specialty Care Team. Collaborates with the nurses in Primary Care and other Specialty Care Clinics. Triages and assesses walk-in patients as necessary. Triages and provides appropriate services to patients who telephone with health concerns. Communicates with home health, hospice, nursing home and non-VA providers and nurses as indicated to assure continuity of patient care. Provides specialty specific direct patient care including, but not limited to: provider ordered follow-ups; chronic disease management; medication administration; vital signs/triage. Provides timely follow-up on any patient complaints/concerns coordinating with providers. Provides patient/family/caregiver education and support and actively promotes patient self-management and personal health responsibility. Coordinates with Non VA Care staff, Non VA Care referrals for veterans. Manages follow-up care via telephone, letter, or My Healthy Vet secure messaging to ensure issues are addressed timely. Coordinates timely notification of normal test results as instructed by the Specialty Care Provider. Assist and coordinates travel and lodging for qualifying veterans that qualify to ensure positive and safe patient outcomes. Participates in quality improvement program activities, clinic specific data collection, and committees. Works in partnership with providers to enhance chronic disease management for a variety of disease processes. Works in partnership with the clinic providers and staff to plan and provide interventions designed to reduce hospital admissions, Emergency Department visits, and walk-ins. Mentors, guides, assists and collaborates with other team members. Performs other duties as assigned.
