The Registered Nurse (RN) CC&ICM Program Coordinator will partner with the CC&ICM Social Worker to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need. The incumbent implements, coordinates, and manages all administrative and clinical aspects of the national CC&ICM framework. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN/ MSN preferred. 3-5 Years experience preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Provides analysis and evaluation of program data and computerized programs to identify system-wide trends and needs to enhance the quality of service across all programs within CC&ICM framework Education, Coaching, Consulting, and Mentoring: Directs/coordinates clinical and psychosocial services and is accountable for the overall effectiveness of the services provided. Modifies services to best meet the treatment needs of veterans and to promote efficient practice. Coordinates Social Work services with other services offered in the treatment program, assuring such services are complementary and comprehensive. Directs activities to maximize effectiveness, efficiency and continuity of care for veterans. Provides case management services to those veterans at high social, psychiatric, or medical risk, serving as the liaison to other treatment programs. Represents the program in contacts with other agencies and the public. Develops and implements policies and procedures specific to Hines and ensures compliance with local facility, VISN, the Associate Director of Patient Care Services (ADPCS), Social Work Service (SWS), VA Central Office(VACO), directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed. Is responsible, in collaboration with Social Work Co-Champion, for the planning, development, implementation, monitoring and evaluation of the CC&ICM operations. Manages and controls the use of data related to this practice regarding the day-to-day functioning and practice evaluation. In collaboration with the Service Chief and/or Supervisory Designee, and Social Work Co-Champion, plans, organized and directs administrative, operational, Veteran and personnel activities for the service. Identifies cohort Veterans across Hines with the highest opportunity/need and potential areas where care coordination is taking place to include but not limited to Primary Care, Surgical Care, Mental Health, Community Care, Specialty Clinics, Geriatrics, and Chronic Disease . Integrates inpatient and outpatient care coordinators, care managers, and case managers for seamless continuity of care. Develops, implement, and monitor process for ongoing screening and identified cohort Identifies care coordinators and support services across the specialized services of Hines and ensure continued participation by setting expectations, clear roles, and responsibilities In collaboration with the Senior Social Work Co-Champion, develops and implement facility wide CC&ICM education plan to key stakeholders to include Service Chiefs, supervisors/managers, frontline staff, and Veterans with plan for ongoing sustainment education plan Coordinates/collaborates with the facility level care coordination and case management teams to include but not limited to nurse and social work navigators, care coordinators, care managers, and case managers Creates a formalized charter for Care Coordination Review Team (CCRT) ensuring representation from all care coordination, care management and case management stakeholders to ensure a seamless and efficient acuity review process for CC&ICM interventions. Coordinates, chairs and co-facilitates the Care Coordination Review Team (CCRT) to integrate communication, collaboration, and coordination when assigning a Lead Coordinator to Veterans identified as needing moderate or complex care coordination services Ensures compliance with policies and standard operating procedures for evaluating and mitigating suicide risk to include: Completion of the Columbia Suicide Severity Risk Scare (C-SSRS), Comprehensive Suicide Risk Evaluation, Safety Plan, and Overdose Report as clinically indicated Conducts psychosocial assessments to determine needs of veterans and family members and is an active participant in the treatment planning process with other disciplines Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
