$2,500 - New grad â�� 1 year relevant experience
$4,000 â�� 2 or more years relevant experience
Sign on Bonus brings with it a 2 year employment commitment, and pays out quarterly over the first year of employment.
Utilizes an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospitalâ��s patients and their families. Performs pre-admission screening review of patients. Assesses and plans interdisciplinary plan of care that effectively manages patient along the continuum. Ensures appropriate use of resources and coordinates discharge planning.

1. RN, licensed in Massachusetts
2. BSN or commensurate experience
3. Computer literacy in Word, Excel, Outlook
1. 5 years clinical experience Required
2. Minimum 1 year Case Management Experience Required
3. Case Management Certification preferred
4. Working Knowledge of InterQual Criteria preferred
5. Outcomes/resources management skills
6. Midas and MediTech Experience preferred
