Northeast Georgia Health System (NGHS) announces an exceptional opportunity for an accomplished leader to serve as Director of Case Management. Anchored by Northeast Georgia Medical Center's three hospital campuses – NGMC Gainesville, NGMC Braselton and NGMC Barrow – with a total of 747 beds and more than 1100 medical staff members representing more than 50 specialties, the health system provides care to more than a million people in more than 18 counties across Northeast Georgia.
Reporting to the Executive Director of Care Coordination, the Director of Case Management maintains system-wide responsibility for aligning patient care coordination with clinical guidelines and organizational standards. Providing leadership to a team of 75 full-time employees, including three direct reports, the Director oversees case management, discharge planning, access control, continuum coordination, pre-certification for post-acute services, resource management, and inpatient acute social work. In addition, the Director will establish, develop, and implement clinical care pathways that enhance cost effectiveness and high-quality care while focusing on system-wide initiatives to improve patient throughput, length of stay, and readmissions.
The successful candidate will demonstrate a minimum of 5 years’ progressive leadership experience in a hospital or health system, with direct oversight leading case management operations for patient care coordination. A Master’s degree in a health care related program, nursing or social work is strongly preferred. Excellent leadership qualities, communication skills, and the ability to establish strong alignment with physicians, nursing, members of the multidisciplinary care team and administration are required.
