The Care in the Community (CITC) Nurse reports to the Chief, Care in the Community and is responsible for providing the care for the Medical Center's CITC Program. The incumbent possesses knowledge of CITC federal regulations, VHA guidelines, clinical documentation requirements, current standards of care and compliance guidelines. This announcement will remain open continuous to fill all hiring needs for CITC. The Registered Nurse demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The Registered Nurse is a proficient nurse and has a full understanding of the patient situation. The Registered Nurse learns from experience what typical events to expect in a given situation and how plans need to be modified in response to these events. The Community Care RN serves as a facilitator for patient care in collaboration with Carl Vinson VAMC clinical and administrative staff and with providers in the community. The incumbent will serve as a clearing house to ensure the most appropriate and economical Purchase Home Community Care (PHCC) is provided, for eligible veterans, either through traditional non-VA care or through the Veterans Choice Program. The incumbent will develop and coordinate the seamless transition of patients throughout the inpatient/outpatient care process, focusing on avoiding unnecessary costs and inconvenience to the patient. The incumbent will serve as the resource person and consultant for problem solving in nursing and organizational issues. The nurse will research available resources within the VA and/or within the veteran's community, keeping in mind the patient's condition and the need for special modes of transportation. The incumbent will actively case manage patients who are hospitalized at Non-VA facilities or residing in a home setting in need of PHCC. The Non-VA Care Nurse is a leader, within the Community Care Program, who is involved in continuous quality improvement, as an approach to assess and improve the quality of health care services, including the utilization of resources.
