***A recruitment/relocation incentive may be authorized for a highly qualified candidate.*** You will provide coordination, planning, and overseeing of the outpatient care for Veterans referred to the Non-VA care program through various avenues of care such as Choice/Health Net and provider agreements. Duties include but are not limited to the following: Screens Non-VA care consults, and obtain pertinent clinical information about the requested outpatient services, determining if the needed care is able to be provided at this facility, and if not, process consult for care in the community using the appropriate avenue and specialty. Communication is made with the community provider and additional records/clinical information are forwarded. Demonstrate competency by utilizing the nursing process not only on an individual level but as a unit/team. Demonstrate leadership by involving others in the improvement of care. Support the client in self-demonstration and act as a patient advocate. Serves as a resource on ethical issues, serve as a preceptor or mentor, participate in quality improvement activities and will participate in education activities to improve their role and expertise. Evaluate self and others using professional standards and will take action to improve performance as an individual and as a unit/team. Use group process to identify, analyze, research, and resolve care problems within their unit. Function in a charge nurse role as needed. Act as a team player and assist other units as needed.
