The primary purpose of the Licensed Vocational Nurse (LVN) Integrated Case Management is to perform assigned duties in a manner that promotes quality health care, position customer service and the maintenance of an environment committed to serving the veteran. The LVN must be astute in caring for the needs of the Veteran struggling with uncontrolled chronic conditions to include but not limited to diabetes mellitus, heart failure, COPD, hypertension, depression, PTSD, and/or other physical and/or mental health conditions. The LVN collects data electronically utilizing home telehealth equipment and modalities. The LVN performs and documents focused assessments following an algorithm template and reports abnormal findings to the Integrated Case Manager for further evaluation and intervention. The LVN demonstrates knowledge and ability to appropriately carry out assigned Veteran care based on the Veterans' conditions. The LVN uses judgment in selecting the appropriate order and sequence of monitoring and treatments. The LVN monitors Veterans' adherence with treatment plans including medications, diet, and exercise. The LVN provides basic education on therapeutic lifestyle changes to aid in chronic disease self-management, including the disease process and importance for adherence with medications, diet, and exercise. The LVN will utilize motivational interviewing techniques in efforts to gain the Veteran's buy-in for their treatment planning. Strong communication skills and dependability of the LVN are essential for Veteran success in Integrated Case Management & Home Telehealth. Preferred Experience: Experience documenting in an electronic health record to include secure messaging, & video tele-health systems Experience working in a team-based environment Successful completion and maintenance of CPR BLS Certification. Medical/surgical background or specialized care Successful completion and maintenance of CPR BLS Certification. High degree of effective oral and written communication skills. Experience using electronic charting systems, include secure messaging, and video tele-health systems Ability to be self-motivated with effective time-management skills. Proficient use of Microsoft system programs (Word, Excel, Power Point, SharePoint, etc.) Work Schedule: Primary Tour of Duty: 0730-1600; the following tours of duty may be scheduled based on organizational needs 0600-1430, 0630-1500, 0700-1530, 0800-1630, 0830-1700, 0900-1730, 0930-1800 Financial Disclosure Report: Not required HPTs who are on track to graduate/complete the requirements should respond as if they have met the requirements.
