M Health Fairview has an immediate opening for an RN to provide patient care at the M Health Fairview Heart Clinic on St. Joe's Hospital campus in St. Paul, MN. This position will provide direct patient care for heart failure patients.
This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening.
This position provides nursing care to patients with cardiovascular disease within the scope of practice for a registered nurse. Care is delivered in
collaboration with the physicians and healthcare team. Evaluates patients, reviews diagnostic tests and initiates/modifies diet, lifestyle
and other therapies using nursing processes.
Consistent, ongoing monitoring, assessment, documentation and personalized education of patients with attention to detail and focus on the patient experience.
Coordination of care with a variety of disciplines and facilities, to maximize patient outcomes and minimize recurrent hospitalizations.
Case Management-style Nursing model with goal-directed care consistent with high quality care.
Facilitate Patient Assistance Medication Programs or Grants available to those meeting guidelines Advocate/assist in referral process for appropriate therapies.
Basic Life Support (BLS) Certification (Red Cross or American Heart Association)
Registered Nurse MN (RN)
Associate's degree in Nursing required.
2 years of post-coronary or coronary critical care experience as a registered nurse.
Knowledge of adult cardiovascular health and disease processes as well as related diagnostic and treatment modalities. Ability to perform a brief history and physical examination and to recognize abnormal findings.
Bachelor's degree is preferred.
Previous heart failure nursing experience is preferred.
