Valley Hospital is seeking a talented Discharge Planner to join our team!
The Discharge Planner works with Utilization Review department on certification and recertification of insurance benefits throughout the patient's stay, and assists the treatment team in understanding the barriers to discharge planning. The Discharge Planner is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team. The Discharge Planner advocates for the patient's access to services during treatment team meetings and through individual physician contact. The Discharge Planner will carry a caseload, will meet with patients daily, be responsible for completing referrals, family sessions, staffing's, and coordinating with outpatient providers.
